Bleomycin

Description:
  • This is a mixture of closely related antitumour antibiotics. It chelates copper or iron, intercalates between DNA strands-causes chain scission and inhibits repair.

Indications:
  • Testicular tumour, squamous cell carcinoma of skin, head & neck, genito-urinary tract & oesophagus, Hodgkin’s lymphoma. Malignancy o lungs & cervix.

Dosage:
  • 30 mg twice weekly in average cases, otherwise dose can be varied between 15 mg daily to 15 mg weekly. Total dose: 300-400 mg.

Contra-Indications:
  • Dot mix glutathione with bleomycin. Pre-existing lung diseases.


Special Precautions:
  • Paediatrics: Safety not established. Pregnancy: Should not be used. Lactation: Should not be used. Elderly: Dose adjustment is necessary.


Side Effects:
  • Mucocutaneous toxicity, pulmonary fibrosis.Dermatitis.Nephrotoxicity.Hepatic toxicity.
  • Drug Interactions
  • Cisplatin enhances the pulmonary toxicity, nephrotoxic agents increases the nephrotoxicity.

Dacarbazine

Description 

Indications:
  • Metastatic malignant melanoma. Hodgkin's disease. Soft tissue sarcomas.

Dosage:
  • Malignant melanoma: 2-4.5 mg/kg/day for 10 days. Hodgkin’s disease: 150 mg/sq m/day for 5 days. Treatment may be repeated after 4 weeks.

Contra-Indications:
  • Pregnancy, lactation, marrow suppression, hypersensitivity.

Special Precautions:
  • Hematological monitoring periodically warranted. Restrict food intake 4-6 hours prior to therapy, Caution in handling-irritant to skin & tissues. Renal & hepatic impairment. Paediatrics: Reduced dose necessary. Pregnancy: Contraindicated. Lactation: Contraindicated. Elderly: Use with caution.


Side Effects:
  • Bone marrow suppression, hepaitc impairment, flu-like syndrome, facial flushing, rash, CNS disturbances, blurred vision, paresthesia, anorexia, nausea, vomiting, alopecia, Budd-chiari syndrome.


Drug Interactions:
  • Dacarbazine may impair immunological response to live vaccine with development of a generalised vaccinia. It forms a precipitate with hydrocortisone succinate.

Docetaxel

Description:
  • It is an antineoplastic agent.

Indications:
  • Cancer : Breast, non-small cell lung, prostate, bladder, stomach, esophagus, overy, head and neck.

Dosage:
  • Administered parenterally. The dose should be individualized depending on the size of the patient and whether or not other medicines are also being taken. Adult:60 to 100mg/1.73m2 intravenously.

Special Precautions:
  • Immunization specifically persons living in the patient's household should not take oral polio vaccine. Paediatric : Only limited studies are done. Pregnancy: Contraindicated. Lactation : Generally not recommended. Elderly : Use with caution.

Leucovorin

Description:
  • Folinic acid (Leucovorin) is an active coenzyme form and does not need to be reduced by DHF Rase before it can act. Methotrexate is a DHF Rase inhibitor; its toxicity is not counteracted by folic acid but antagonised by folinic acid.

Indications:
  • Antidote for methotrexate & folic acid antagonists.
Dosage:
  • Dose equal to antagonist given: to be given within 1 hr. Usually ineffective if given after 3 hrs.
  • Contra-Indications
  • ernicious anaemia and megaloblastic anaemia secondary to the lack of vit B12.


Special Precautions:
  • Undiagnosed megaloblastic anaemia, folate dependent tumors. Paediatrics: Use with caution. Pregnancy: Safety not established. Lactation: Saferty not established. Elderly: May cause severe G.I. disturbances and other adverse effects.


Side Effects:
  • Allergic sensitization, nausea, pyrexia, vomiting, urticaria, anaphylactic reactions.


Drug Interactions:
  • Enhances cytotoxic effect of fluorouracil. Reduces methotrexate toxicity. Phenobarbitone, phenytoin, primidone counteracts the antiepileptic effects.

Methotrexate

Description:
  • It has a cell cycle specific action-kills cells in S phase; primarily inhibits DNA synthesis, but also affects RNA and protein synthesis. It blocks the convesion of a essential coenzyme required for one carbon transfer reaction in de novo purine synthesis and amino acid inter conversions.

Indications:
  • Lymphoblastic leukaemia. Choriocar-cinoma.

Dosage:
  • Lymphoblastic leukaemia: 20-40 mg/sqm. body surface area twice weekly i.v or i.m. Maint dose: 15-30 mg/sqm 1-2 times weekly. Chorio carcinoma: 15-30 mg daily for 5 days.

Contra-Indications:
  • Severe leucopenia or thrombocytopenia. Serious anaemia

Special Precautions:
  • Use of folinic acid rescue has permitted much higher doses of methotrexate. Hepaticand renal dysfunction. C.N.S. disturbance. Bone marrow depression. Paediatrics: Use only in cancer treatment. Pregnancy: Contraindicated due to its teratogenic effects. Lactation: Should not be used. Elderly: Use with caution.

Side Effects:
  • Megaloblastic anaemia, pancytopenia, Desquamation and bleeding may occur in G.I.T.


Drug Interactions:
  • Salicylates, sulphonamides, phenytoin, henylbutazone, tetracyclines, chloramphenicol and para-aminobenzoic acid increase toxicity. Asparginase reduces toxicity.

Paclitaxel

Description:
  • It is a diterpenoid compound with anti-tumour activity. It binds specifically to the b-tubulin subunit of microtubule and appears to antagonise the disassembly of this cytoskeletal protein.


Indications:
  • Metastatic ovarian and breast cancer.


Dosage:
  • 175 mg per square metre by iv infusion over 3 rs. Repeat every 3 weeks. Pretreat with corticosteroids,antihistaminics and H2 antagonists.

Contra-Indications:
  • Hypersensitivity.


Special Precautions:
  • Monitor haematological functions. Paediatrics: Not recommended. Pregnancy: Not usually prescribed. Lactation: Not advised.

Description:

Temozolomide

Description:
  • Temozolomide is an imidazotetrazine alkylating agent with antituour activity. It undergoes rapid chemical conversion in the systemic circulation at physiological pH to the active compound, monomethyl triazeno iidazole carboxamide (MTIC). The cytotoxicity of MTIC is thought to be due promarily to alkylation at the O6 Position of guanine with addtional alkylation also occurring at the N7 Position. Cytotoxic lesions that develop subsequently are through to involve aberrant repair of the methyl adduct.

Indications:
  • Patients with newly diagnosed glioblastoma multiforme concomitantly with radiotherapy and then as adjuvant treatment. Patiets with recurrent high grade glooma, such as glioblastoma multiforme or anaplastic astrocytoma, patients with advanced metastatic malignant melanoma.

Dosage:
  • Concomitant phase consists of Temozolomide adiministered orally at 75mg/m2 daily for 42 days with focal adiotherapy (60Gy administered in 30 fractions) The concomitant phase is followed by the adjuvant phase (Temozolomide for 6 cycles).

Contra-Indications:
  • Contraindicated in the case of hypersensitivity, in pregnancy, in Lactation & in patient with severe myelosuppression.

  • Administration of Temozolomide with ranitidine did not result in clinically significant alterations in the extent of absorption of Temozolomide. Co-administration of dexamethasone, prochlorperazine, phenytoin, carbamazepine, ondansetron, H2-receptor antagonists or phenobarbital did not alter the clearance of Temozolomide. Topotecan

Description: Topotecan inhibits topoisomerase I, an enzyme involved in DNA replication.

Indications:
Metastatic carcinoma of the ovary after failure of initial or subsequent chemotherapy. Small cell lung cancer disease after failure of first line chemotherapy.

Dosage:
Before administration of topotecan, patient should have a baseline neutrophil count of > 1500 cells/mm3 and a platelet count of >100000 cell/mm3. The recommended starting dose is 1.5mg/m2 by IV infusion over 30 minutes daily for 5 consecutive days, starting on day 1 of a 21 day course. In absence of tumour progression a minimum of four courses are recommended, as tumour response may be delayed.

Drug Interactions Other antineoplastic agents : Myelosuppression may be more severe if topotecan is given with cisplatin. G-CSF : Coadministration of G-CSF and topotecan may prolong duration of neutropenia. If G-CSF has to be used, it should be started only on day 6 of the course i.e., 24 hr. after completion treatment with topotecan.
